1.        New Member:  The safety committee welcomes Joe Zimmerman as a new member of the safety committee representing the front office.

 

2.        Incidents:

 

  • Release of epoxy fumes from MSM-173 resulting in evacuation of area.  No prior notice received from Planning and Construction due to annual leave
  • Fumes from welding of HVAC on roof, no notification received
  • Paint fumes, notification received
  • Roof work, release of non hazardous fumes during non working hours, prior notification received

 

3.        Inspections:  In October, NV Dept. of Environmental Protection will be randomly inspecting HRC labs for proper chemical storage, waste storage, waste Satellite Accumulation Area* (SAA) upkeep.  Notify George Fratus (EH&S) within the next 10 days of any waste or chemicals you want disposed of in preparation for the NDEP inspection.

 

* Satellite Accumulation Area (SAA) is defined as a location at or near any point of generation where dangerous waste is initially accumulated in containers before consolidating the waste at a designated accumulation area (90/180-day) or storage area.  The term “satellite accumulation area” is not found in the federal Resource Conservation and Recovery Act (RCRA) regulations, 40 CFR.  Each satellite accumulation area is limited by volume, up to either 55 gallons of dangerous waste or one quart of acutely hazardous waste.

 

Fire Safety:  The Nevada fire marshal will be on campus starting November 1st.  In preparation for that visit, Ed Gannon (EH&S) accompanied by Klaus Stetzenbach (executive director) will be inspecting for fire hazards and violations about two weeks prior.  Safety committee members will be notified so that they can each accompany Ed & Klaus (in their respective areas) on these inspections.  At that time, if you have any “door propping questions” bring them up with Ed so that we can come to final resolution of these problems.

 

Please keep the following in mind:

  • If there are sprinklers, need an 18” clearance below
  • Do not remove rated doors.  Any doors that have been removed will be put back on hinges (DNA lab)
  • Store away flammables and combustibles in proper designated storage areas
  • Do not prop rated doors, when in doubt, don’t prop them**
  • 55 Gallon Drum Issue***

 

        ** Door Wedges (http://ehs.unlv.edu/forms/serviceguide.pdf):  The use of wedges or door stops is a violation of the fire code and strictly prohibited. Office doors are typically rated at one hour and are designed to prevent a fire rom spreading into adjacent areas.  Keeping the door open prevents the door from doing its job. The State Fire arshall will enforce this specific fire code with a citation of $1000 issued to the individual who placed the wedge or door stop to keep the door open. When a student is visiting your office for counseling or other situations when the door must be held open, the State Fire Marshall will permit the use of a chair, waste paper basket or other large similarly sized object. The rationale is that a large object is more easily seen and moved so that the fire door will close.

 

*** 55-gallon drums in MSM-181 (herpetology) need to have secondary containment (drip pans).  George offered to bring in a drum lifter to insert the containment or we can transfer the materials to smaller easier to handle drums.  The smaller drums do not need the secondary containment.

 

OSHA:  Aurali Dade (EH&S) conducted an internal inspection of HRC.  Jeanette Daniels (Chair) has discussed the results with the appropriate HRC representative.  These reports are attached to the minutes as the Appendix.

 

Firearms:  Public safety, Luiz Ferreira, inspected the gun storage in (building and room number omitted) and interviewed ornithology personnel.  He found no problems with the storage and security.  Firearms procedures are in review and permission to store and transport in UNLV vehicles was granted, provided ornithology personnel are trained to the policies and procedures; firearms inventory is filed with Public Safety; and the inventory is kept up to date.  In addition he recommended an alarm system be installed on the room housing firearms. 

 

4.        Training: 

 

        Cart Training:  Anyone driving a cart needs to take cart training with Jimmy Maffessanti (EH&S).  They then need to take a driver’s test with Michelle Bossler.

 

OSHA Hazwoper refresher training:  Generators of hazardous waste are required to be trained (when at a large quantity generator facility). To implement this requirement, 29 CFR 1910.1200 is an applicable training standard to use, if applied properly. Thus, per UNLV procedure, people who manage SAA are required to take this training.  Because there are approximately 5 SAAs in the building, the 3 people who are currently trained, Jeanette, Klaus, and Amy, are considered adequate for the time being.  A 24-hour course will soon be offered to replace the 40-hour training, so more personnel may be trained at that time.

 

5.     Facility Concerns:

 

All non-routine maintenance is to go through Klaus.

 

Intrusion Alarms:  We are looking at alarming a number of areas of the building.  Including, the nuclear/chem. labs, micro labs and the bird range firearm storage area.  Please see letter from the Chief of Police.  This is a way to track who is going in and out of these areas during a specified time.

 

Keys:  Although it may seem that many areas of the building have not had key problems, several masters have been misplaced or not returned.  As a consequence, we are going to rekey the entire building.  The Division Directors will be making the decision on how they want their areas keyed and who is to have which keys.  The key issue has turned out to be much more complex than initially planned.   Submit any changes in keying to Jeanette.  The front office and wooden external doors will be Marlok accessible.

 

General: 

 

a) In the next few months we will need to get rid of the old file cabinets in the hallway outside Klaus's office.  The Archaeology group has gone through and removed their items.  If the front office can go through the rest of the cabinets to ensure nothing of importance is thrown away, that would be appreciated.

 

b) Facilities will be removing their items in the electrical storage room in MSM-176 and that will revert to storage space for the HRC/MSM.  However, it is an electrical room so we have to see what the State Fire Marshall will allow us to store in there.  It would be a great area for Archeology to store equipment and artifacts as they are almost full in the current area and will be adding to their collection in the near future. Perhaps more than one group could share the area.

 

c) The HVAC that will cool room 176 is currently being installed and the installation should last another few days.  This is about a $15,000 unit that was kindly paid for by facilities and did not cost HRC/MSM anything.

 

d) Archeology has removed the display cases in the hallway outside Klaus's office.  That space will be temporarily used by the Nuclear group to store lab cabinets from Lab 173 (rad lab) while a new floor is being installed.

 

e) Lab 173 (rad lab) is being remediated for an asbestos tile floor replacing it with acid-proof flooring.  This is the same lab that has recently been remodeled.  There is a slight contract problem with the remediation company that is causing a delay in the floors being done.  We are still hopeful the floor job can start in the next couple of weeks or so.

 

f) Once the floor is finished in lab 173, the supplies and equipment from the foyer area by the North entrance glass doors will be moved into the labs and that area can then be made into a professional looking reception area for visitors who enter the building. Also, the supplies that are being stored in 271 will be moved into the labs opening up that space.  That area is proposed as a document storage area for the building.

 

g) Lab MSM-165 (general use lab) will be upgraded soon.  This project was voted on by the Executive Committee and is being paid for by the HRC. The lab will have to be cleared of all chemicals and some equipment and that lab will be shut down for a maximum of a month.  It is unsure where the temporary storage of those supplies will be.

 

h) The first floor bathrooms in the hallway across from Klaus's office are being completely remodeled including, but not limited to, ventilation, flooring, walls, doors, fire alarms, electrical and plumbing.  This is a pretty big project and will involve outside contractors in the building.  Facilitiesis finalizing plans with the trades and that project should start in October.  We hope to have that finished within a month or so, but it will be done by Christmas for sure. This project is being paid fully by the University and is not costing the HRC/MSM any money.

 

i) The painting on the third and fourth floors of the HRC is still not completed.  The paint shop has been pulled away for other emergency projects, but will finish the paint job in the near future.  Please let Jeanette know if anyone wants their office painted (it will shut you down for a day and you need to move everything off the walls).

 

j) The IDR for the front office conference room upgrade is being submitted today.  This is a fairly simple project.  The bookshelves will be cut partially down to allow for an 18" fire sprinkler clearance.  A projection screen will be installed above the bookshelves and the room will be painted. This was voted on by the Executive Committee and is being paid for by the HRC/MSM.

 

5.        Other business:  A question was raised whether there is a policy concerning the use of cell phones in the laboratories and if not should the safety committee establish one.  There is no existing policy.  The chair, Jeanette said she would discuss the issue with Klaus.

 

6.        Automatic External Defibrillator:  John Taylor (EH&S) stated that the UNLV policies and procedures governing AED use are in the final approval stage.  Upon approval, HRC will be furnished with copies and training by him and Jimmy Maffessanti will be provided.

 

7.        Next Meeting:  The next meeting may be held after the fire marshal inspection, depending on the findings of that inspection.  Otherwise, although the goal is for quarterly meetings, they will be held on more of an “as-needed” basis.

Appendix-Results of OSHA Inspection

 

 

 

As part of an ongoing effort by UNLV to protect employees and to comply with Federal and

State requirements, the following laboratories were inspected on July 19, 2004. The requirements

for this inspection are outlined in OSHA regulation 29 CFR 1910.1450 and also in the OSHA

required UNLV Chemical Hygiene Plan. Under that plan the Chemical Hygiene Officer is charged to

perform regular inspections of all laboratories.

 

The following is an easy to read formdetailing those items that require correction. Please note as a reminder: all safety equipment must be free of obstructions; this includes safety showers, eyewashes, fire extinguisher,

electrical panels, etc.

MSM-422

 

Items to be Corrected as Soon as Possible:

Documentation of internal safety training and procedures for handling hazardous materials

must be initiated. This should include a written record of laboratory staff’s signatures when

trained on safety aspects of new hazardous procedures and/or materials. Please see attached

form (Aurali can e-mail this to you); you may use this form or develop your own.

Items to be Corrected as Soon as Possible:

Storage of any item must be at least 18 inches below the bottom of the sprinkler heads, also

heavy, hazardous or glass items should not be stored high overhead.

Food & drink should only be stored & consumed in designated areas.

MSM 165, 167, 172A, 306

 

Items to be Corrected as Soon as Possible:

All potential tripping hazards in walkways in 172A such as electrical cords should be

removed.

Emergency phone numbers should be readily visible (306).

All refrigerators should have warning signs, please see attached sign for an example.

The safety shower in 306 is blocked. Obstructions should be removed.

Items corrected by Environmental Health & Safety

The fire extinguisher in 306 should be mounted. This was passed on to fire safety.

 

 

 

 

MSM 164, 168

 

Items to be Corrected as Soon as Possible:

All refrigerators should have warning signs, please see attached sign for an example.

Staff members were unable to find a first aid kit at the time of the inspection. A first aid kit

should be present in the laboratory. Please see attached information on recommended first aid

kits for purchase.

 

 

MSM 146, 208 & 410

 

Items to be Corrected as Soon as Possible:

Storage of any item must be at least 18 inches below the bottom of the sprinkler heads, there

were some boxes stored in 146 that were above this level.

There was some expired ether in 410, this information was passed on to Hazardous Materials

Safety.

There were missing ceiling tiles in 410, this information was passed on to Fire Safety.
